Cornerstone VNA Welcomes New Hospice Care Director
Rochester, NH – Cornerstone VNA, a trusted nonprofit home health provider, is pleased to announce the appointment of Lisa Nichols, RN, MSN, CCM, IQCER, as the new Hospice Care Director. Lisa brings a strong background in nursing leadership, case management, and quality patient care. She is excited to further strengthen Cornerstone VNA’s mission of providing compassionate and comprehensive care to patients and their families.
Lisa is a Registered Nurse, with a Master’s degree in nursing, and a certificate in Executive Leadership from Perdue University Global. She is a certified InterQual instructor and a certified case manager, bringing a wealth of expertise in training, developing, and motivating nursing teams to deliver top-quality patient care. Lisa is known for her supportive and empowering leadership style and has a proven track record in policy development, cost-saving initiatives, and effective teaching. Lisa is also an active member of several professional organizations, including the Case Management Society of America, the American Case Management Association, and Sigma Theta Tau International Honor Society of Nursing.

Her vision for the future of hospice care at Cornerstone VNA includes a focus on continuous improvement, patient education, and fostering a compassionate care environment for both patients and their families.
